Document Checklist and Instructions

A properly documented loan application makes your loan process go smoothly. This checklist will help you gather your paperwork.

  1. ANSWER ALL QUESTIONS (EVEN THOUGH IT SAYS REQUIRED ONLY) and sign the loan application.  If applying with another borrower be sure and sign at the top of page one in addition to the third page.  At the end of the application after you select the "submit" button you will have the option to print out the application and disclosures.  Sign all forms and fax to us at (785) 273-9559.

Documentation needed if you want to proceed beyond a pre-approval.

  1. If you are a salaried or hourly employee: provide the W-2 for the past year and the two most recent paystubs.  Paystubs must be computer generated, indentify the employee and employer, contain year-to-date information, and include at least 30 days of income. If you are self-employed, provide  tax returns for the past two years, including all schedules.
  2. If you own rental property, provide recent rental agreements and tax returns for the previous two years, including all schedules.
  3. To speed up the approval process, provide bank statements for the most recent two months, and recent statements for stock, mutual funds and IRA/401K accounts.
  4. If you are requesting a cash out refinance, provide a letter explaining how you will use the refinance proceeds, whether the house has been listed for sale within the last 12 months and proof the listing for sale has been cancelled. 
  5. If applicable, provide a copy of your divorce decree and settlement agreement.  Child support must have at least a good 12 month payment history to be counted as income and must be current.  This applies to those receiving or paying child support or maintainence.
  6. If you are NOT a US citizen, provide your green card to be copied. If you are NOT a permanent resident provide your H-1 or L-1 visa.
  7. If any borrower has filed bankruptcy, provide the Discharge Notice, Filing and Schedule of Creditors.
  8. If you are applying for a home equity type of loan, please provide proof of insurance, county appraisal, mortgage or deed, and most recent paystub.
